Personal information you disclose to us

Briefly: We collect personal information that you provide to us voluntarily.

Upon electing to register with us or utilize our services,  personal information that you voluntarily provide of your own volition is collected for the purpose of fulfilling the requested services or activities.

The personal information collected is dependent on the extent and nature of the requested services, information, or activity requested and agreed to by you and at your discretion

The collected personal information may  include the following:

  • Publicly Available Personal Information. We collect first name, maiden name, last name, and nickname; current and former address; phone numbers; email addresses; social media; and other similar data.
  • Personal Information Provided by You. We collect passwords; financial information (credit card number, purchase history, invoices); and other similar data.
  • Payment Data. We collect data necessary to process your payment if you make purchases, such as your payment instrument number (such as a credit card number), and the security code associated with your payment instrument.   • PHI (Protected Health Information as defined by HIPAA may include (but not limited to):

Geographical identifiers smaller than a state, except for the initial three digits of a zip code if, according to the current publicly available data from the U.S. Bureau of the Census: the geographic unit formed by combining all zip codes with the same three initial digits contains more than 20,000 people; and the initial three digits of a zip code for all such geographic units containing 20,000 or fewer people is changed to 000

Information automatically collected

Briefly: Some information — such as IP address and/or browser and device characteristics — may be recorded  automatically when interacting with our site. (See Cookie Policy Notice)

Automated collection of  certain information when you visit, use or navigate our site and related resources . The information automatically collected does not disclose precise and specific identity such as name or contact details but may include device, IP address, browser, operating system, location to include country, city, state, referral sources and links, site use behaviors, and related technical data. The primary uses of such information is site security, operational efficiency, reporting, and analysis to improve our services.

We do or may collect information through cookies and comparable processes as is standard practice in electronically accessed business portals.

 

Online Identifiers. We collect cookie identifiers, or others such as the ones used for analytics and marketing; device’s geolocation; tools and protocols, such as IP (Internet Protocol) addresses; and other similar data.

Information collected through our Apps

Briefly: We may collect information regarding your geo-location, and Facebook permissions when you use our apps.

If you use our Apps, we may also collect the following information:

Geo-Location Information. We may request access or permission to and track location-based information from your mobile device, either continuously or while you are using our mobile application, to provide location-based services. If you wish to change our access or permissions, you may do so in your device’s settings.

Facebook Permissions. We may by default access your Facebook basic account information, including your name, email, gender, birthday, current city, and profile picture URL, as well as other information that you choose to make public. We may also request access to other permissions related to your account, such as friends, checkins, and likes, and you may choose to grant or deny us access to each individual permission. Briefly: Information acquired is for the sole purpose of delivering and fulfilling consensually requested (by you) services, activities, or pursuits.

When available, and to facilitate account creation and logon process. If you choose to link your account with us to a third party account (such as your Google or Facebook account), we use the information you allowed us to collect from those third parties to facilitate account creation and logon process for the performance of services that you have requested.

 

To send you marketing and promotional communications. We and/or our third party marketing partners may use the personal information you send to us for our marketing purposes, if this is in accordance with your marketing preferences. To post testimonials. We may post testimonials provided regarding our Services that may contain limited personal information such as First Name, Last Initial, City and/or State. Prior to posting a testimonial, we will obtain your consent to use your name and testimonial. Request Feedback. We may use your information to request feedback and to contact you about your use of our Services.

To protect our Services. We may use your information as part of our efforts to keep our Services safe and secure (for example, for fraud monitoring and prevention).

To enable user-to-user communications. We may use your information in order to enable user-to-user communications with each user’s consent.

To enforce our terms, conditions and policies for Business Purposes, Legal Reasons and Contractual.

To respond to legal requests and prevent harm. If we receive a subpoena or other legal request, we may need to inspect the data we hold to determine how to respond.

To manage user accounts. We may use your information for the purposes of managing our account and keeping it in working order.

To deliver services to the user. We may use your information to provide you with the requested service.

To respond to user inquiries/offer support to users. We may use your information to respond to your inquiries and solve any potential issues you might have with the use of our Services.

We may use your information for other Business Purposes, such as data analysis, identifying usage trends, determining the effectiveness of our promotional campaigns and to evaluate and improve our Services, products, marketing and your experience. By using our Maps API Implementation, you agree to be bound by Google’s Terms of Service. You agree to allow us to obtain or cache your location. You may revoke your consent at anytime.

 

  1. HOW LONG DO WE KEEP YOUR INFORMATION?

 

Briefly: We keep your information for as long as necessary to fulfill the purposes outlined in this privacy policy unless otherwise required by law.

 

We will only keep your personal information for as long as it is necessary for the purposes set out in this privacy policy, unless a longer retention period is required or permitted by law (such as tax, accounting or other legal requirements). No purpose in this policy will require us keeping your personal information for longer than 1 year past the start of idle period of the user’s account.

When we have no ongoing legitimate business need to process your personal information, we will either delete or anonymize it, or, if this is not possible (for example, because your personal information has been stored in backup archives), then we will securely store your personal information and isolate it from any further processing until deletion is possible.

 

  1. HOW DO WE KEEP YOUR INFORMATION SAFE?

 

Briefly: We aim to protect your personal information through a system of organizational and technical security measures.

 

We have implemented appropriate technical and organizational security measures designed to protect the security of any personal information we process. However, please also remember that we cannot guarantee that the internet itself is 100% secure. Although we will do our best to protect your personal information, transmission of personal information to and from our Services or Apps is at your own risk. You should only access the services within a secure environment. Although we are not a “Covered Entity” under HIPAA, we strive to comply with the same high standards of security and protection of private information as dictated by HIPAA due to the nature of the services typically requested and provided.

What data breach procedures we have in place

In case of a data breach, our system administrators will go through affected users and will attempt to reset passwords if needed after informing the user. We will create a fresh backup, scan the site for malware and attempt to identify the breach, removing any associated code and means of access. Subsequently we will look at preventative measures that will prevent a similar breach from happening again, and take action where possible.

We also use protocols and standards described by HIPAA as general guidance in responding to a data breach. These standards may be viewed at  https://www.hhs.gov/hipaa/for-professionals/breach-notification/index.html It is important to note that we are not a “Covered Entity” under HIPAA and not legally bound to these standards. As such, we instead use the standards as a guide and not all elements will apply. We will follow the spirit and intent of the guidance with specific focus and attention to definitions and courses of action as they apply to affected users. Notification to regulatory authorities will be made in accordance with applicable requirements as a Non Covered Entity and not as a Covered Entity. The objective is full compliance with any data breach to the extent required by law and with actions guided by higher standards wherever applicable. This voluntary standard shall not be interpreted to in any other manner than as an understanding of our commitment to voluntarily applying to the extent possible and practical the “guidance” of HIPAA and not to any extent legal requirement beyond that required by My MD Bill Review as a Non Covered Entity under HIPAA.

 

  1. DO WE COLLECT INFORMATION FROM MINORS?

 

Briefly: We do not knowingly collect data from or market to children under 18 years of age.

 

We do not knowingly solicit data from or market to children under 18 years of age. By using the Services, you represent that you are at least 18 or that you are the parent or guardian of such a minor and consent to such minor dependent’s use of the Services. If we learn that personal information from users less than 18 years of age has been collected, we will deactivate the account and take reasonable measures to promptly delete such data from our records. If you become aware of any data we have collected from children under age 18, please contact us at support@mymdbillreview.com. Knowingly engaging our services in violation of this requirement is a violation of our terms of use and terms of service and grounds for immediate account revocation. Additionally, you accept responsibility for such violations and we may in accordance with legal requirements refer such violations to any appropriate authorities as required by law.

 

  1. WHAT ARE YOUR PRIVACY RIGHTS?

 

Briefly: You may review, change, or terminate your account at any time.

 

We only provide services to clients with healthcare billing related needs related to care provided and billed by medical providers in the United States. Knowingly engaging our services in the absence of these requirements is a violation of our terms of service and terms of use. If you are resident in the European Economic Area and you believe we are unlawfully processing your personal information, you also have the right to complain to your local data protection supervisory authority. Account Information

If you would at any time like to review or change the information in your account or terminate your account, you can:

  1. CONTROLS FOR DO-NOT-TRACK FEATURES

No uniform technology standard for recognizing and implementing DNT signals has been finalized. As such, we do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online. If a standard for online tracking is adopted that we must follow in the future, we will inform you about that practice in a revised version of this privacy policy.

Briefly: Yes, if you are a resident of California, you are granted specific rights regarding access to your personal information.
